Quality Measures

What is measured?

Baptist Health Physician Partners is completely focused on quality. As a partner, you will receive a Quality Performance Summary each quarter that contains: Physician Demographics, Attribution Summary, Engagement Measures/Results, Quality Measures/Results, and a Star Ranking among peers in your specialty.

Below, you will find the different quality groupings for physicians and their respective measures for 2017.

Measure Definition:

The portion of prescribed drugs within the therapeutic class that were generic as categorized by Medispan. Percent Generic is calculated as the number of 30-day adjusted prescriptions filled for generic and over-the-counter drugs divided by the total number of 30-day adjusted prescriptions. “% of Generic Usage” is the percent of generic prescriptions that a given prescribers writes in the selected drug class. “Cost Per Unit (Avg)” is the average cost of a prescription in a given drug class for a particular provider.

Outpatient Discharging Group

Outpatient physicians who discharge from a Baptist Health facility with 24+ cases per year.

How are patients attributed?

Crimson is a claims-based data repository. Physicians practicing at a Baptist Health facility that are listed as the last attending in Epic will be attributed as the discharging physician in Crimson. All consultants and proceduralist charge data will also be submitted to Crimson as it is reflected on the Epic claim.

Measure Definitions:

Average Length of Stay for Outpatient Procedures: Displays the arithmetic average length of stay for a case. As a large percentage of hospital outpatient cases are reimbursed by a fixed amount, appropriate length of stay is increasingly important. Monitoring trends can identify situations where length of stay can be reduced without sacrificing patient safety.

Percentage of Outpatient Procedures with an Emergency Department Visit within 3 Days: Displays the percentage of outpatient procedure cases that are followed by an emergency department visit within 3 days. Only cases that were treated and released from the ED are considered for this measure. The percentage is based on the total number of outpatient procedures performed, not the total number of ED visits.

Only cases that were treated and released from the ED are considered for this measure. Physicians that Admit, Discharge, Consult or Perform Procedures at a Baptist Health Facility

  • Generic Drug Utilization of 84% or greater, partnership wide.
  • Length of Stay
  • Percentage of 30 Day Readmissions with Excludes (Any APR-DRG)
  • Mortality Rate with Excludes
  • Hospital Acquired Conditions (HACS/1,000 patients)

Emergency Room Physicians

  • Log into Crimson Analytics (Continuum of Care) account quarterly and view available data.
  • Download and engage the Crimson Connect Communication App.
  • Generic Drug Utilization of 84% or greater, partnership wide.
  • Length of Stay in Hours
  • Percentage of 72 Hours Revisits to ED
  • Percentage of 72 Hour Revisits to ED Resulting in an Inpatient Admission

Primary Care Physicians (Attributed to Patients Under BHPP Contracts)

  • Log into Crimson Analytics (Crimson Population Risk Management) account quarterly and view available data.
  • Download and engage the Crimson Connect Communication App.
  • Generic Drug Utilization of 84% or greater, partnership wide.
  • Avoidable Inpatient Admissions
  • % of 30 Day Readmissions (All Cause)
  • Evidence Based Measure Compliance for:
    • Breast Cancer Screening
    • Diabetes Hemoglobin A1C Monitoring
    • LDL Screening for Patients with Diabetes, CAD, Hyperlipidemia, or diagnosis CVA/TIA
  • ED Utilization/1,000 Members
